Documents and forms related to our 2026 summer season are linked below. They cover overall wellness at Betsey Cox, safety requirements, and the sign on forms you will need to bring with you opening days. Please reach out with any questions.
If you choose to the sign on forms ahead either by USPS or electronically, please plan to bring the originals with you on opening day. They will be collected in person by our health care team that day, and sometimes forms can lost in transit! Thank you.

- ACA Health History Part I
- Families should complete information in Part I pages 1-3. Page 4 is for arrival day and our face to face check in. Immunization records from your camper’s primary care physician can be attached.
- ACA Health Care Recommendations Part II
- Signed by a Primary Care Physician (Often Primary Care Physicians prefer to use their own forms for Part II. We can accept those forms as long as it is attached to the ACA Part II. A school physical has all the same information we need at Camp.)
